Can I choose any SAP?

Yes, you can choose your SAP provider. Make sure they are DOT-qualified (I am).

Can I get a second opinion?

No. Once you begin the SAP process with one provider, DOT regulations prohibit seeking a second SAP's evaluation. Choose your SAP carefully.

Will my employer know the details?

I will provide your employer with required reports (violation type, recommendations, compliance status, follow-up testing plan), but detailed clinical information remains confidential.

How much do SAP evaluations cost?

Initial SAP evaluation is $400 and follow-up evaluation is $200. SAP evaluations are employment-compliance services and are not covered by insurance. Payment is due at time of service. Credit/debit cards accepted. Bundled rate is $500 if you pay for both evaluations upfront.

Can I do the evaluation remotely?

Yes! DOT accepts telehealth SAP evaluations. I offer both in-person and virtual options.

What if I don't complete the recommended treatment?

You cannot return to safety-sensitive duties until you demonstrate successful compliance with recommendations.

How many follow-up tests will I need?

Minimum of 6 unannounced tests in the first 12 months. Testing can continue up to 60 months based on my clinical recommendation.
